D & C at 12 weeks after seeing fetal hb twice(dd ment)

After an ectopic pg (scarred/bad left tube from surgery) and 3 early m/c, I had my dd (4 years) on GonalF, Ovidrel, Progesterone & Baby Aspirin.
We started trying for our second child a year ago under the same RE. After 6 failed cycles of GonalF, Ovidrel, Prog, BA, we got pg in June. I had 2-3 big cysts on week 6 but there was a small one by week 9. Saw the fetal h/b on week 6 and 9 and then during my first OB appt visit after 12 weeks, the baby had no h/b. OB estimated the h/b stopped in week 10. Had D & C done 2 days later.

I had gestational diabetes when I was pregnant with dd.
I gained about 15 lbs since I we started ttc, since I was advised by RE against workout due to increased and enlarged follicles with GonalF.
My blood group is A -ve and DH has AB +ve and was given Rhogam shot after dd's birth.
I had a series of immunological tests done all came back normal.
So what went wrong? What are my future options?
