Dr. Cusick..your thoughts on the NK cell /Cytokines theroy?

Posted by Tateen on 11:38 6/2/2005:

Do you test for this in your patients? Do you believe in IVIG treatments? I am just curious on your thoughts on this.
I have been pg 3 times. The first one ended in m/c. My next pregnancy was SEVERLY complicated by SCH! I started bleeding at 6 weeks. I would pass TONS of hug clots and bleed and bleed. At 13 weeks I was dx with a huge SCH in my uterus and chances are I would loose the pg. Well I then found Dr. Beer(RI doctor). He said NK cells were high and we did aggressive tx with IVIG. THat pg went to 38 weeks and I had a healthy baby!
My next pg I had a NK cell before conception and it was normal. When I got pg I started bleeding again at 6 weeks. It stopped at 14 weeks and then I started Lovenox. It was found that I have +APA and positive for the prothrombin gene mutation. I went ahead with IVIG treatment also. Even though my NK cells were normal through the pg...they said my Cytokines were high. I again delivered a healthy baby at 37 weeks.
We want to TTC again. My perinatologist believes that if I do basa and Lovenox right away with a postive pg test that would be enough for me to sustain a pg and not bleed. He does not feel I need to see and RI and does not think NK cell testing and cytokines is necessary.
I am hoping he is right but scared to death of another SCH and loosing my baby to NK/cytokines cells.
Just curious what you would do/think about this.
Thanks
Kathleen Castrovillari
