my risk

I am not pg yet, but am trying with meds. I have pcos, ir and hypothyroid. My thyroid has been regulated from the start, I am on metformin 2000xr, and using follistim and hcg shot for fertility. I am 33, 5'2" and 200 lbs. I eat well and exercise little (I know I need to do more), my cholestrol is great, 105 bad and 89 good, c-reactive good, a little high, b/p 126/86 (fluctuates by only a few points). My gp said whatever I am doing to keep doing it, my b/w is very good. I know I am going to be in the high risk in pg, and I will need to see a specialist when the time comes. My nurse/midwife isnt able to give me the info about what makes me a high risk exactly (she said the thyroid but that was it). I want to know what I will need to pay attention to, change before pg, durring pg, and what exactly puts me in the high risk?
