High risk preg? need suggestions

have lupus and am trying to conceive. I have + ANA titer 1:320 but DS-DNA is currently negative. Also have autoimmune hypothyroidism and just started Synthroid 25mcg; retest TSH in 4w. Any other testing needed before ttc again? How often should Rheum see me? How often would I need to see OB and mat-child specialist? Do have a previous child and they did serial u/s monthly to check for iugr. I had preterm labor at 35 most likely do to my huge fibroid but baby was ok. Thanks for your comments and assistance!
