Question for the doctor (pg/de/child/preemie ment)

Doctor, I am 13w5d pg with one donor egg baby (donor is age 30). I have two issues. First, I have been bleeding red then brown since about 5 or 6 weeks, but it always stops on its own and u/s has not shown the cause. Usually it is a small amount of red followed with a couple of days brown spotting, a couple of days clear, then it starts over again. I am on Lovenox 40mg 2x/day (reduced from 60mg 2x/day b/c of the bleeding) due to diagnosis of hetero Factor V Leiden and hetero Prothrombin G mutation after my son's birth (IUGR and clot in cord) at 30w5d - he weighed 1 lb. 10 oz. Pre-term birth was indicated rather than spontaneous.

Second issue - I recently had NT scan with blood work and although the scan was fine (1.7 mm, baby measuring on target, good h/r) the Free Beta HCG was .24 MoM and PAPP-A was .36. This puts me at increased risk of Trisomy 18/13 - risk assessed at 1:161. Upon further research I found that these blood results can also point to a placental insufficiency that could result in IUGR again. I am scheduled for another u/s at 16w to check for Trisomy 18 or 13 markers. I would prefer to avoid amnio, but would do it under some circumstances.

Questions:

1) My OB tells me that the bleeding puts me at greater risk for pre-term delivery/P-PROM - why is that? I understand that the two are assoicated, but is the causation known? Baby has always seemed fine, thank God.

2) Based on my history I would guess (and actually hope) that the 1st trimester blood results are due to a placenta issue rather than a trisomy. Based on my history and the fact that this is a 30-year old donor egg, do you have any thoughts on this?

3) If next u/s looks good, would you recommend the amnio anyway? I am very nervous about it b/c of the bleeding plus all the bad luck I have had (also have had two early m/cs).

Sorry so long - thanks for your input. I see OB next week and just want to have the right questions for her - are there any others I should ask?
