Strange ultrasound report (infant death mentioned)

I delivered twins at 21w3d in Nov. Today I received my hospital records, and found something very confusing in the ultrasound report. The scan was done when I was 20w1d, and the report says that twin B's head circumference was 104mm, corresponding to 50w. Twin A's was 177mm, corresponding to 20w. Obviously this makes no sense.
Also, we had an ultrasound done at 18w and the twins were growing concordantly. From what I can tell, the measurement of 104mm would correspond to much earlier than 18 weeks- if there had been such a discrepancy wouldn't it have shown up on the 18w ultrasound?
I'm wondering if this means there's been an error.
