We don't use a cut-off for NT- based on size of the fetus. Your measurement is a little bigger than average, however the vast majority of these kidos are fine. Your NT probably won't alter your age related risk
Test is best when combined with bloodwork (and other U/S measures)- blood work could/can reduce your risk significantly
At worst, the test would tell you what you already know- your risk for Down syndrome is greater than a 35 year old's risk
