Bleeding and Lovenox - Anticardiolipin and MTHFR

Posted by ausviz2 on 22:01 1/15/2008:

Hi Bill-
I'm currently 10w pregnant with twins. At 8w, I had some light bleeding/spotting that has since gone away. During an early u/s, they noted that I had a small SCH (5 mm by 6 mm by 7 mm) that has since healed and isn't visible on u/s.

Due to a history of recurrent pregnany loss, I consulted with a hematologist to look over all my RPL tests. The only thing that has been abnormal is that my anticardiolipin IgM has always been slightly elevated in the low end of the indeterminate zone. The hematologist did some more specialized tests and found that my anticardiolipin IgM jumped up to low positive while pregnant. He also found that I'm compound heterozygous C677T/A1298C for MTHFR. My homocysteine levels are normal.

Due to these two tests, he put my on a low dose of lovenox (40 mg 1x per day). He kept warning me that lovenox may increase my risk of bleeding and cause me to lose the pregnancy. From what I've read, the risks seem to be very low, especially since I seem to be so good at losing pregnancies on my own. What are your thoughts on this?

Also, he told me no baby aspirin while on the lovenox. It seems like most people stay on the baby aspirin when starting the lovenox. What do you usually do when starting someone on lovenox?

One last question. Do you think I started this protocol too late? Or is there still a chance that things could work out? I saw the 2 h/b's yesterday and they both look good, although both babies were measuring a day behind.

Thanks!
Kristin
